Please make sure all forms are completely filled out and turned into the front desk prior to your child starting any of our child care programs.
Additional Preschool Paperwork
If your child is enrolled in our preschool program, we will also need a copy of all of the following things:
- Shot record
- Physical signed by their physician
- All four and five year old's - vision and hearing screening from their physician
Please make sure these are turned into the front desk prior to your child starting our preschool program.
